How our Protocol Works
Our alcohol use disorder treatment combines three evidence-based components under the supervision of Dr. Truman Milling, MD, FACEP, a board-certified emergency physician:
1. Ongoing Psychotherapy
Treatment begins and continues with regular therapy sessions. Using a trauma-informed lens, our clinicians work with you to understand the root causes driving alcohol use — whether that's unresolved trauma, chronic stress, anxiety, depression, or something else. EMDR therapy may be incorporated to help process underlying traumatic experiences that contribute to the pattern of use.
2. Physician-administered Ketamine Infusion
A series of three weekly intravenous ketamine infusions is administered by Dr. Milling in our private West Austin offices. Ketamine creates a window of heightened neuroplasticity — a period during which the brain is more receptive to forming new neural pathways. This window is leveraged through the ongoing therapy that follows, helping to disrupt ingrained patterns and build healthier thought and behavior loops.
Ketamine is an FDA-approved medication (Schedule III, approved since 1970). Its use for alcohol use disorder is considered off-label and is supported by a growing body of clinical evidence. All infusions are administered intravenously at 0.7 mg/kg over 40 minutes, with full medical monitoring throughout.
3. Naltrexone
Following the ketamine infusions, clients receive a monthly injection of Naltrexone. Naltrexone works by blocking the opioid receptors responsible for the endorphin "reward" associated with drinking. With the reward response diminished, clients find drinking less appealing, and new neural pathways formed within the metaplastic window of the ketamine infusions can take hold more effectively.
The combination of these three elements — therapy addressing the root cause, ketamine opening a window for neural change, and Naltrexone removing the chemical reward of drinking — creates a reinforcing cycle that supports sustained recovery.
